Thank you for donating an auction item! - Please fill out this form completely. The required fields are marked with a red star.
- If the donation is from a Business, please fill out the Business Name field, and make the Address, City and State be the Business's address.
- The fields marked optional may be left blank.
- If you are donating more than one of the exact same item to be auctioned separately and not as a package, please put the number of the items you are donating in the Quantity field, otherwise put 1 in the Quantity field. If you are donating a pair of tickets as a package to an event, then put 1 in the Quantity field.
- If you are donating more than one different items, please use this form multiple times for each different item.
- Remember to email the image(s) for each item you are donating to auctionitems@walthamlandtrust.org; more details at the end of the form.
